If COC report is of certain category (i.e. Medal) then show a block which allows the request to be raised in a way that makes the issuing easier.

Description

If they have a higher attachment, then don't allow the awarding of a lower attachment
If they already have a medal, update the existing entry, don't create a new one
move all schema to live db
add a view requirements link when medal assigned
BUG if they have the medal but no attachment it does not detect it correctly
Allow for putting a reason in the PAR to be picked up by the function
